Bay District Schools is committed to ensuring that all student-athletes have a safe and successful experience participating in extracurricular athletics. Before any student can participate in workouts, practices, tryouts, or contests, they must complete the athletic clearance process through AthleticClearance.com and receive approval from their school. To help families navigate the process, we have created an Athletic Clearance Document Checklist that outlines the required forms based on each student's educational setting.

Documents Required for All Students

Every student-athlete must complete and upload the following:

  • EL2 – Preparticipation Physical Evaluation

  • NFHS Concussion in Sports Certificate

  • NFHS Heat Illness Prevention Certificate

  • NFHS Sudden Cardiac Arrest Certificate

  • NFHS Sportsmanship Certificate

Additionally, beginning July 1, 2026, Florida law requires an ECG/EKG screening (EL1) for all incoming ninth-grade student-athletes and any student in grades 10-12 who have never previously participated in interscholastic athletics. (fhsaa.com)

Traditional Public School Students

Students attending their zoned Bay District School and participating in athletics at their enrolled school must upload:

  • EL1 (New to High School Athletics)

  • EL2

  • NFHS Concussion in Sports Certificate

  • NFHS Heat Illness Prevention Certificate

  • NFHS Sudden Cardiac Arrest Certificate

  • NFHS Sportsmanship Certificate

Home Education Students

Home education students are eligible to participate in athletics at their assigned public school but must provide additional documentation to verify enrollment and academic eligibility. (Mandarin High Athletics)

Required documents:

  • EL1 (New to High School Athletics)

  • EL2

  • EL7/EL7V

  • GA4

  • Official Grade Record

  • Bay District Schools Extracurricular Activity Commitment Form

  • NFHS Concussion in Sports Certificate

  • NFHS Heat Illness Prevention Certificate

  • NFHS Sudden Cardiac Arrest Certificate

  • NFHS Sportsmanship Certificate

Personalized Education Program (PEP) Students

Students participating through Florida's Personalized Education Program (PEP) must submit:

  • EL1 (New to High School Athletics)

  • EL2

  • EL15

  • GA4

  • Official Grade Record

  • Bay District Schools Extracurricular Activity Commitment Form

  • NFHS Concussion in Sports Certificate

  • NFHS Heat Illness Prevention Certificate

  • NFHS Sudden Cardiac Arrest Certificate

  • NFHS Sportsmanship Certificate

Charter, Collegiate, Private, Florida Virtual School (FLVS), Bay Virtual, Alternative, or Traditional Public School Students Participating at Another School

Students who attend one of these educational settings and are participating under Florida's school choice and non-traditional student provisions must upload:

  • EL1 (New to High School Athletics)

  • EL2

  • EL16

  • GA4

  • Official Grade Record

  • Bay District Schools Extracurricular Activity Commitment Form

  • NFHS Concussion in Sports Certificate

  • NFHS Heat Illness Prevention Certificate

  • NFHS Sudden Cardiac Arrest Certificate

  • NFHS Sportsmanship Certificate

Before You Submit

Please verify the following before submitting your athletic clearance:

  • All forms are fully completed and signed

  • All uploaded documents are legible

  • Physical examination is current and valid

  • Official grade documentation has been uploaded (if required)

  • All NFHS course certificates have been uploaded

  • All required FHSAA eligibility forms have been uploaded based on student status

Important Reminder

Students are not eligible to participate in workouts, practices, tryouts, camps, or contests until all required documentation has been submitted, reviewed, and approved through Athletic Clearance. Families are encouraged to begin the process early to avoid delays when sports seasons begin.
